Case Manager

Athena Behavioral Health is inviting applications for the position of Case Manager to support patient coordination, counseling, documentation, and multidisciplinary clinical care in our mental healthcare facilities.

Key Responsibilities

• Meet patients daily in the morning to review progress and concerns.
• Maintain detailed understanding of each patient’s medical, psychological, and emotional condition.
• Prepare and present patient reports during IPD rounds.
• Ensure all decisions and actions are properly documented and communicated to the Senior Resident.
• Support accurate and timely diagnosis in coordination with the healthcare team.
• Provide psychoeducation to patients to support recovery and awareness.
• Develop and implement individualized discharge plans.
• Conduct individual and family counseling sessions.
• Act as the coordination bridge between doctors, patients, families, and nursing staff.
• Ensure seamless communication across all aspects of patient care.
